    Philosophical foundations of the Death and Anti-Death discussion

    Perhaps there has been no greater opportunity than in this “VOLUME FIFTEEN of our Death And Anti-Death set of anthologies” to write about how might think about life and how to avoid death. There are two reasons to discuss “life”, the first being enhancing our understanding of who we are and why we may be here in the Universe. The second is more practical: how humans meet the physical challenges brought about by the way they have interacted with their environment. Many persons discussing “life” beg the question about what “life” is. Surely, when one discusses how to overcome its opposite, death, they are not referring to another “living” thing such as a plant. There seems to be a commonality, though, and it is this commonality is one needing elaboration. It ostensibly seems to be the boundary condition separating what is completely passive (inert) from what attempts to maintain its integrity, as well as fulfilling other conditions we think “life” has. In our present discussion, there will be a reminder that it by no means has been unequivocally established what life really is by placing quotes around the word, namely, “life”. Consider it a tag representing a bundle of philosophical ideas that will be unpacked in this paper

    The ontology of number

    What is a number? Answering this will answer questions about its philosophical foundations - rational numbers, the complex numbers, imaginary numbers. If we are to write or talk about something, it is helpful to know whether it exists, how it exists, and why it exists, just from a common-sense point of view [Quine, 1948, p. 6]. Generally, there does not seem to be any disagreement among mathematicians, scientists, and logicians about numbers existing in some way, but currently, in the mainstream arena only definitions, descriptions of properties, and effects are presented as evidence. Enough historical description of numbers in history provides an empirical basis of number, although a case can be made that numbers do not exist by themselves empirically. Correspondingly, numbers exist as abstractions. All the while, though, these "descriptions" beg the question of what numbers are ontologically. Advocates for numbers being the ultimate reality have the problem of wrestling with the nature of reality. I start on the road to discovering the ontology of number by looking at where people have talked about numbers as already existing: history. Of course, we need to know not only what ontology is but the problems of identifying one, leading to the selection between metaphysics and provisional approaches. While we seem to be dimensionally limited, at least we can identify a more suitable bootstrapping ontology than mere definitions, leading us to the unity of opposites. The rest of the paper details how this is done and modifies Peano's Postulates

    Scholars have wrestled with consciousness , one writer calling it the hard problem . Some thirty-plus years after the Towards a Science of Consciousness, we do not seem to be any closer to an answer to What is consciousness? . Seemingly irresolvable metaphysical problems are addressed by bootstrapping, provisional assumptions, not unlike those used by logicians and mathematicians. I bootstrap with the same ontology and epistemology applicable to everything we apprehend. Here, I argue for a version of the unity of opposites, a form of neutral monism. Something exists because of what it is not; nothing can exist by itself, singularities (analogous to monads), expressing the relationship. Applying this most fundamental law to the consciousness problem, the physical (movement) comes into being simultaneously as the mental (stasis), each displayed by a field that is variegated (ranging from creative to entropic processes). Such is in keeping with how the singularity emerged as our universe, the same form that may account for sub-Planck-scale phenomena. Overall, like the most fundamental law, deep structures are imminent in our universe, both as objects and processes

    Evaluation of ADAM-12 as a diagnostic biomarker of ectopic pregnancy in women with a pregnancy of unknown location

    Ectopic pregnancy (EP) remains the most life-threatening acute condition in modern gynaecology. It remains difficult to diagnose early and accurately. Women often present at emergency departments in early pregnancy with a 'pregnancy of unknown location' (PUL) and diagnosis/exclusion of EP is challenging due to a lack of reliable biomarkers. Recent studies suggest that serum levels of a disintegrin and metalloprotease protein-12 (ADAM-12) can be used differentiate EP from viable intrauterine pregnancy (VIUP). Here we describe a prospective study evaluating the performance of ADAM-12 in differentiating EP from the full spectrum of alternative PUL outcomes in an independent patient cohort.Sera were collected from 120 patients at their first clinical presentation with a PUL and assayed for ADAM-12 by ELISA. Patients were categorized according to final pregnancy outcomes. Serum ADAM-12 concentrations were increased in women with histologically-confirmed EP (median 442 pg/mL; 25%-75% percentile 232-783 pg/mL) compared to women with VIUP (256 pg/mL; 168-442 pg/mL) or miscarriage (192 pg/mL; 133-476 pg/mL). Serum ADAM-12 did not differentiate histologically-confirmed EP from spontaneously resolving PUL (srPUL) (416 pg/mL; 154-608 pg/mL). The diagnostic potential of ADAM-12 was only significant when 'ambiguous' PUL outcomes were excluded from the analysis (AROC = 0.6633; P = 0.03901).When measured in isolation, ADAM-12 levels had limited value as a diagnostic biomarker for EP in our patient cohort. The development of a reliable serum biomarker-based test for EP remains an ongoing challenge

    A group of Scottish care leavers visited Denmark to undertake a comparative piece of research looking at the similarities and differences between care and leaving care provision in the respective countries. They interviewed Danish young people in two projects for care leavers and visited a range of other projects for young people in care. Whilst the lived experience of the two groups was broadly similar in relation to common themes of loss, stigma and levels of support in making the transition into young adulthood, the researchers found that the quality of relationships with staff/carers for the Danish young people was often better. The number of placement moves whilst in care was much lower for the Danes. The overall material and nurturing provision for Danish children and young people impressed the researchers

    Shotgun Proteomics Identifies Serum Fibronectin as a Candidate Diagnostic Biomarker for Inclusion in Future Multiplex Tests for Ectopic Pregnancy

    Ectopic pregnancy (EP) is difficult to diagnose early and accurately. Women often present at emergency departments in early pregnancy with a 'pregnancy of unknown location' (PUL), and diagnosis and exclusion of EP is challenging due to a lack of reliable biomarkers. The objective of this study was to identify novel diagnostic biomarkers for EP. Shotgun proteomics, incorporating combinatorial-ligand library pre-fractionation, was used to interrogate pooled sera (n = 40) from women undergoing surgery for EP, termination of viable intrauterine pregnancy and management of non-viable intrauterine pregnancy. Western blot was used to validate results in individual sera. ELISAs were developed to interrogate sera from women with PUL (n = 120). Sera were collected at time of first symptomatic presentation and categorized according to pregnancy outcome. The main outcome measures were differences between groups and area under the receiver operating curve (ROC). Proteomics identified six biomarker candidates. Western blot detected significant differences in levels of two of these candidates. ELISA of sera from second cohort revealed that these differences were only significant for one of these candidates, fibronectin. ROC analysis of ability of fibronectin to discriminate EP from other pregnancy outcomes suggested that fibronectin has diagnostic potential (ROC 0.6439; 95% CI 0.5090 to 0.7788; P>0.05), becoming significant when 'ambiguous' medically managed PUL excluded from analysis (ROC 0.6538; 95% CI 0.5158 to 0.7918; P<0.05). Fibronectin may make a useful adjunct to future multiplex EP diagnostic tests
